New research suggests that a dual sensory impairment significantly raises the chances of developing dementia.

Researchers have linked hearing impairment and vision impairment individually to an increased chance of developing dementia. However, a new study finds that an individual’s chances of developing dementia are significantly higher when they have both conditions.

The risk of developing dementia increases by 86% for individuals who have both hearing and visual impairment.

“Evaluation of vision and hearing in older adults may predict who will develop dementia and Alzheimer’s. This has important implications for identifying potential participants in prevention trials for Alzheimer’s disease, as well as whether treatments for vision and hearing loss can modify risk for dementia.”

The research, “Dual sensory impairment in older adults and risk of dementia from the GEM Study,” appears in Alzheimer’s & Dementia: Diagnosis, Assessment & Disease Monitoring.

According to the study, some 33% of people aged 70 and older experience hearing loss, and vision loss affects about 18% of people in this age group. Since these conditions worsen with age, researchers think that a correlation exists between the advance of these conditions and the loss of a person’s functionality and mortality.

Previous research found a less obvious connection between hearing or visual loss and the onset of dementia.

The two leading theories indicate that either these specific impairments result from similar physical processes that cause dementia, or that hearing and visual loss lead to an increasing sense of social isolation, depression, and physical inactivity, each of which may lead to dementia.

The new study looks at the impact of having both conditions together. It refers to this combination as “dual sensory impairment.” According to the study:

“Although most prior studies have focused on impairments in hearing and vision individually, the impact of having combined hearing and visual impairment, or dual sensory impairment (DSI), on dementia risk is unclear.”

To investigate the impact of DSI on developing dementia, the researchers analyzed data collected by the Gingko Evaluation of Memory (GEM) study, a double‐blind randomized‐controlled trial investigating the value of Ginkgo biloba for preventing dementia in older people.

The GEM study involved adults aged 75 or older who, at the outset of the study, had normal cognitive function, or at most, mild dementia. The researchers studied the participants for 8 years.

In addition to collecting self-reported data regarding the individuals’ hearing and vision, follow-up examinations of participants assessed the development of dementia.

The final data set that Hwang and his colleagues studied consisted of 2,051 individuals. Of these, 1,480 people had neither hearing nor visual loss. 14.9% reported visual impairment, and 7.8% reported hearing loss. Just 5.1%, or 104 individuals, had DSI.

Compared to participants who did not have hearing or visual loss, those reporting DSI were more likely to be male, have other comorbidities, and describe themselves as smokers and drinkers.

The GEM study’s follow-up examinations revealed the following likelihoods of developing dementia:

  • 14.3% of those without any reported hearing or vision loss developed dementia.
  • 16.9% of those reporting a single impairment developed dementia.
  • 28.8% of those with DSI developed dementia.

People with both hearing and visual loss were nearly twice as likely to develop dementia as those without such impairments.

However, according to the results, the increased risk of dementia for an individual with DSI is only somewhat related to the severity of hearing and visual impairment.

While those classified as having a high level of DSI are at the greatest risk of developing dementia, those with lower DSI levels also have a significantly higher risk of dementia.

The researchers analyzed the link between DSI and three types of dementia: all-cause dementia, Alzheimer’s-related dementia, and vascular dementia.

The chances of acquiring Alzheimer’s’-related dementia are even higher. The study indicated that people with DSI are 112% more likely to develop Alzheimer’s than those without any impairment.

Hwang and his colleagues found no association between DSI and vascular dementia.

The study’s conclusions suggest that doing more to mitigate hearing and visual loss in older people may be a way to hold off or thwart the onset of dementia.

The authors conclude, “Further research is needed to characterize the exact role of sensory impairments and whether treatments that improve sensory function, such as surgery or sensory aids, devices, and prostheses, can modify this risk.”

The study’s authors add that shifts toward an older general population make addressing the prevalence of dementia in the elderly more urgent.

They write: “Because the public health burden of dementia will increase over the next three decades, evaluation of vision and hearing function in older adults may help identify patients at elevated risk of developing dementia.”

Scientists have discovered a way to track changes in sound processing in the brains of people with HIV. Their test provides a simple technique for studying how HIV affects the central nervous system.

By measuring brain signals, researchers have observed the impact of HIV on the central nervous system.

Due to advances in antiretroviral drug treatment over the past 20 years, most people with HIV can now expect to live long, healthy lives.

However, even if treatment successfully brings the virus under control, people can experience cognitive issues as a result of HIV causing damage to their central nervous system.

Up to 45% of people with HIV may develop these difficulties, which are known collectively as HIV-associated neurocognitive disorder (HAND).

Although the cause of HAND remains unknown, scientists have several theories. They speculate, for example, that it may be the result of chronic inflammation, lingering damage from the initial infection, or a toxic effect of antiretroviral drugs.

Alternatively, low levels of the virus may persist in the brain as a result of the blood-brain barrier limiting the passage of antiretroviral drugs into the brain.

The signs and symptoms of HAND can be subtle, which makes it challenging for doctors to diagnose early and monitor.

One of the problems that people with HIV sometimes report is difficulty understanding speech in the presence of background noise.

Researchers at the Geisel School of Medicine at Dartmouth in Hanover, NH, led a team that studied the hearing of a group of people with HIV in Dar es Salaam in Tanzania.

They collaborated with scientists from the Muhimbili University of Health and Allied Sciences in Dar es Salaam and neuroscientists from Northwestern University in Evanston, IL.

“Initially, we thought we’d find that HIV affects the ear, but what seems to be affected is the brain’s ability to process sound,” says study co-leader Dr. Jay Buckey Jr., a professor of medicine at Geisel.

To test this idea, they used scalp electrodes to monitor the brain waves of 68 people with HIV and 59 people without as they listened to sounds.

Specifically, they recorded a brain response known as the speech-evoked frequency-following response (FFR).

Certain features of brain waves, such as their timing and amplitude, reveal how faithfully the brain encodes an auditory stimulus. The FFR has proven particularly useful for investigating how the brain processes information in highly complex sounds, such as music and speech.

The precision and accuracy of the FFR can improve as a result of experience, such as in trained musicians, or worsen as a result of illness or damage, such as concussion.

“There are many acoustic ingredients in speech, such as pitch, timing, harmonics, and phrase,” says study co-leader Nina Kraus, Ph.D., a professor of communication sciences and neurobiology at Northwestern.

“The FFR enables us to play speech sounds into the ear of study participants and figure out how good a job the brain is doing processing these different acoustic ingredients.”

The researchers recorded the FFR while the study participants listened to common speech sounds, such as “ba,” “da,” and “ga.”

They looked at how well the signal encoded two acoustic features of speech, known as the harmonics corresponding to the first formant and fundamental frequency.

The harmonics corresponding to the first formant identify the units of sound, or “phonemes,” that differentiate one word from another, whereas the fundamental frequency reflects the frequency at which the vocal cords vibrate, which helps identify the speaker.

The brains of people with HIV had similar responses to the brains of people without HIV at processing the fundamental frequency.

They also performed well on standard tests of hearing, but the FFR revealed that the brains of people with HIV were worse at encoding the first formant.

In other words, they had normal hearing thresholds, but their brains could not distinguish between the sounds of different words as accurately.

“When the brain processes sound, it’s not like a volume knob where all of the acoustic ingredients are either processed well or poorly,” Kraus explains. “With the FFR, we’re able to see which aspects of auditory processing are affected or diminished and ask, ‘[I]s there a specific neural signature that aligns itself with HIV?’”

The researchers have published their study in the journal Clinical Neurophysiology.

The researchers note that as people get older, even if their hearing remains normal, their brains get worse at processing the same speech features that the brains of people with HIV seem to struggle with.

They write that HIV-associated cognitive problems may therefore be akin to early aging.

In the future, they hope that other scientists will use the FFR not only to study brain dysfunction associated with HIV, but also other conditions, such as concussion and Alzheimer’s disease.

Standard tests of cognitive function involve giving people tasks, such as solving math problems or remembering a list of words. However, these are dependent on the language people speak and their culture.

“What’s significant about our results is that the test doesn’t require any actions on the [subject’s] part. It’s recorded passively — subjects can even sleep or watch a movie,” says Dr. Buckey. “We think the FFR holds a lot of promise as a way to assess the brain easily and objectively.”

In their paper, the researchers note that the test could prove particularly useful in parts of the world with the most HIV infections and limited healthcare resources.

They write, “While previous neuroimaging and electrophysiological studies have shown differences between HIV+ and HIV- individuals, these approaches are difficult to transport to resource-limited settings such as sub-Saharan Africa, where >70% of the world’s HIV+ population lives and where there is some evidence that neurocognitive symptoms are more severe.”

“Thus, the FFR holds promise as a research tool to further study [central nervous system] health, particularly in resource-limited settings.”

The study authors acknowledge that research projects involving more participants will be necessary to confirm their results. They say that it would also be interesting to track changes in the FFR from the early stages of infection onward.

Finally, in light of evidence suggesting that different strains of the virus may have differing neurological effects, they would like to see studies that compare FFR tests in different populations around the world.

Is the legal battle over for Michael Flynn?

Judge Emmet Sullivan delays all proceedings in the Michael Flynn case after an appeals court orders the case be thrown out; Judge Andrew Napolitano weighs in on what happens next.

U.S. District Judge Emmet Sullivan on Thursday ordered the cancellation of a hearing he had scheduled in Michael Flynn‘s case, following a ruling from the D.C. Circuit Court of Appeals saying Sullivan has to dismiss the case.

The Department of Justice had filed a motion to dismiss Flynn’s case despite his guilty plea after unsealed FBI notes cast doubt on the bureau’s intentions when they interviewed Flynn in January 2017. Sullivan refused to grant the motion right away. Instead, he called for briefs and a hearing to debate the matter, even appointing retired Judge John Gleeson to argue against the dismissal.

STRZOK NOTES SHOW OBAMA, BIDEN WEIGHED IN ON FLYNN CASE EVEN AS COMEY DOWNPLAYED IT: LAWYERS

Flynn then petitioned the D.C. Circuit for a writ of mandamus — which would order Sullivan to drop the case — which the court granted in a 2-1 decision.

Sullivan’s decision to cancel a hearing, which had been scheduled for July 16, along with other related deadlines, does not necessarily mean that he is ready to dismiss the case against Flynn, who was accused of providing false statements to the FBI related to his contact with a Russian ambassador. Sullivan could opt to challenge the ruling by seeking a review from the full D.C. Circuit Court roster.

After initially pleading guilty, Flynn changed course and accused the FBI of acting improperly when they interviewed him in the early days of the Trump administration. The circumstances surrounding the FBI’s interview of the then-national security adviser were further called into question when handwritten FBI notes indicated that there was debate over whether investigators wanted to get the truth from him or get him to lie so he could be pressured by the possibility of prosecution or termination.

FLYNN-KISLYAK CALL TRANSCRIPTS RELEASED, REVEALING FATEFUL TALKS OVER RUSSIA SANCTIONS

Flynn was ultimately both charged and fired.

This week, new notes, purportedly handwritten by former FBI official Peter Strzok, were turned over to Flynn’s attorneys during a Justice Department review of the case. Flynn’s lawyers then filed the notes as part of his criminal case. The notes raised further questions about who was involved in the Flynn investigation, as Flynn attorney Sidney Powell claimed in a court filing that they showed that then-President Barack Obama called for “the right people” to investigate Flynn, while then-FBI Director James Comey appeared to have said that Flynn’s calls with the ambassador “appear legit.”

Powell said the notes also seemed to indicate that it was then-Vice President Joe Biden who suggested that the Logan Act — a constitutionally questionable law from 1789 that has never been successfully used in prosecution — be used as the reason for the questioning.

It is not clear if the notes were intended to say what Flynn’s lawyers interpreted in their filing.

A House Judiciary Committee hearing focused on oversight of the Department of Justice was marked by searing accusations and explosive interruptions from the very beginning, as Democrats and witnesses accused Attorney General Bill Barr and his agency of putting politics over the rule of law.

In his opening statement, Committee Chairman Rep. Jerrold Nadler, D-N.Y., excoriated Attorney General Bill Barr — who was not at the hearing, but is set to testify next month — accusing him of a “clear and dangerous pattern of conduct” of shielding President Trump’s associates and undermining cases stemming out of the Russia investigation.

BARR TO TESTIFY BEFORE HOUSE JUDICIARY COMMITTEE NEXT MONTH

Nadler cited the DOJ’s motion to dismiss Michael Flynn’s criminal case, but ranking member Rep. Jim Jordan, R-Ohio, pointed out in his opening statement that earlier in the day the D.C. Circuit Court of Appeals agreed that Flynn’s case should be dismissed and ordered Judge Emmet Sullivan to toss the case.

Jordan also defended Barr for speaking out against actions that took place under the Obama administration during the Russia probe. He noted that Nadler would not call DOJ Inspector General Michael Horowitz to testify before the committee after Horowitz wrote in a lengthy report that FBI officials made significant omissions and inaccurate representations to the FISA court in seeking a warrant to monitor a former Trump campaign adviser.

Nadler also brought up the DOJ’s sudden decision to recommend a lighter sentence than originally suggested for former Trump associate Roger Stone. Andrew Zelinsky, a prosecutor who resigned from the case after being overruled by DOJ superiors, testified at the hearing that the recommendation to go below the traditional sentencing guidelines was politically motivated.

“Roger Stone was treated differently because of politics,” Zelinsky said, adding, “I was told that the acting United States Attorney was giving Stone a break because he was afraid of the president of the United States.”

APPEALS COURT ORDERS FLYNN CASE DISMISSAL, AFTER YEARS-LONG LEGAL SAGA

Stone, in February, was sentenced to three years in prison, after being convicted in November 2019 on seven counts of obstruction, witness tampering, and making false statements to Congress on charges that stemmed from former Special Counsel Robert Mueller’s investigation.

Federal prosecutors had recommended a prison sentence of 87 to 108 months for the charges, but in a reversal, senior leadership at the Justice Department, including Barr, overruled and scaled back their recommended prison sentence.

Zelinsky stated that he had no issue with the sentence itself, rather it was the process that bothered him and led him to leave his position.

JUDGE NAPOLITANO: PROBES OF TRUMP ASSOCIATES ‘WILL PROBABLY CONTINUE’ AFTER FIRING OF SDNY FEDERAL PROSECUTOR

The hearing erupted with commotion during the opening statement of the first witness, former Deputy Attorney General Donald Ayer, who exceeded the five-minute time limit while blasting Barr as posing “the greatest threat in my lifetime to the rule of law.” Despite Republicans noting that Ayer had gone over the time limit, and Rep. Louie Gohmert, R-Texas, repeatedly banging on a table so he would stop, Ayer kept going with Nadler granting him extra time. Former ranking member Doug Collins, R-G., took Nadler to task for “capriciously” deciding how to apply the rules, but Nadler ignored his concern.

Nadler later allowed former Attorney General Michael Mukasey to go over the time limit during his opening statement.

Mukasey defended Barr, pointing out that despite accusations he is carrying out a political agenda, he allowed Mueller to finish his investigation and declined to prosecute former FBI deputy director Andrew McCabe, who was found to have made misrepresentations in statements regarding FBI leaks related to an FBI investigation of Hillary Clinton.

Mukasey insisted that Barr’s decisions are not politically motivated and that rather he “makes decisions based on relevant facts and the law.”

A DOJ spokesperson confirmed Wednesday that Barr himself will testify before the same House committee on July 28.

“They got tricked! They got led astray! They got bamboozled! And they are, respectfully, Madam Chair, mad as hell,” Espaillat said. “I am mad as hell!”

The New York Democrat who represents Upper Manhattan and part of the Bronx said he could “literally count on one hand” the small business owners who did get help under the CARES Act. He blamed this on improper use of the program and called for those responsible to be held accountable.

“This is a major problem. It is absolutely not how any of intended for this program to work,” he said. “The actors who perpetrated this must be investigated and they must face consequences.”

The meeting came ahead of a full House vote on another stimulus bill that would provide additional funds to keep the program going so more businesses could apply.

Espaillat suggested that moving forward, more local lenders should be allowed to participate so that business owners who do not have relationships with larger financial institutions can get help.

“We must do better,” he said. “We must bail out Main Street!”
