“Well, I don’t know where the six times comes from. We need to be careful, I think, about making comparisons to flu. This infection hasn’t been as prevalent in children as flu is each year, there’s been about 330,000 diagnosed infections,” Gottlieb said. “If you believe we’re diagnosing one in five to one in 10 infections in children, maybe there have been about three million kids who’ve been infected with this.”
Gottlieb conceded that coronavirus did not appear to be “as prevalent” in children compared to the flu. However, he noted that in addition to some 90 children’s deaths from the virus, there are indications that it may cause inflammatory post-viral syndromes in children in some case.
“So there’s a lot we don’t understand about COVID in kids. I think we need to be careful about making comparisons to flu and the death and disease we see in flu relative to COVID,” Gottlieb said.
The former FDA commissioner also acknowledged that while he had expected a peak and decline in deaths by this point, “there’s been a fairly persistent level of infection, hospitalizations and deaths over the last couple of weeks.”
“We’ve had over a thousand deaths a day for at least two weeks now, over 50,000 infections a day on average,” he said. “We hit 55,000 in the last day. Hospitalizations have come down a little bit, but they haven’t really started to decline very rapidly.”

]]>Washington — Dr. Scott Gottlieb, former head of the Food and Drug Administration, warned Sunday it’s going to be “very hard” for school districts to open for the upcoming academic year if there is uncontrolled spread of the coronavirus within their communities.
“For local school districts, I think they need to be looking at what is the spread within the community. If you have uncontrolled spread within the community, it’s going to be very hard to open against that backdrop.” Gottlieb said on “Face the Nation.”
With the fall semester set to begin in the coming weeks, the Trump administration had been urging school districts to open their doors to students for in-person instruction five days a week. But the push from President Trump and Education Secretary Betsy DeVos comes as Mr. Trump warned last week the coronavirus crisis will likely “get worse before it gets better” and acknowledged school districts in hot spots may have to delay reopening for a few weeks.
Still, the Centers for Disease Control and Prevention issued guidance Thursday for how best to safely open schools for students. The guidance calls for schools to follow social distancing, recommend students and staff wear face coverings, and keep students in defined groups to reduce contact with others, among other guidelines.
But the CDC did not provide a benchmark for school administrators on when they should move to close a school in the event of an uptick in coronavirus infections, which Gottlieb said was “unfortunate.”
“In the setting of uncertainty and the lack of specific guidance for how to keep schools open, I think more parents are going to err on the side of caution,” he said. “That’s why it’s very important to get specificity out of what the hard metrics are that the CDC didn’t do in its guidance.”
Gottlieb said school districts should think about positivity rates in their community, local spread and testing capacity to guide decision-making on whether to keep a school open or shift fully to online learning. A positivity rate between 5% to 10%, he said, is “starting to get iffy,” while above 10% is the “threshold where you really want to think carefully about closing the school districts.”
“That is a sign that there is an epidemic underway inside that community,” Gottlieb said, adding that it’s going to be difficult for parts of Florida, which is an epicenter of the coronavirus epidemic in the U.S., to open their schools for in-person learning next month.
More than 30 states and the District of Columbia are experiencing an uptick in coronavirus infections, with the epicenter of the epidemic currently in the Sun Belt. But Gottlieb said there are indications cases in some states including Arizona and Texas are beginning to plateau.
While Gottlieb said there are “unmistakable signs” the epidemic is slowing in some states across the Sun Belt, it’s “heating up” in others, including South Carolina, Alabama, Indiana and Illinois.
Many governors have instituted mandates requiring people to wear masks when they are in public spaces and halted their phased reopenings in an effort to mitigate the spread of the coronavirus. Gottlieb said states should consider shutting down bars, limiting the number of people in restaurants, closing movie theaters and other kinds of indoor entertainment venues, and implementing mask mandates to quell the epidemic, steps that allow states to stop short of reverting back to the more stringent restrictions imposed at the start of the pandemic in March.
In addition to states issuing mask mandates, a number of businesses are requiring customers to wear face coverings as well. McDonald’s is among the latest to require face coverings, and the fast food giant joins Walmart, Starbucks and others in implementing their own rules for masks.
Gottlieb said he expects more businesses will implement mask mandates in venues, especially those considered high-risk.
“If we really can manage to keep the epidemic under control heading into the winter until we get to a vaccine or some kind of therapeutic that changes the clinical trajectory of this illness, just with masks that’s going to be something relatively simple that we can all do that doesn’t really change our lives and allows us to maintain what’s really important to us like keeping some businesses open, like getting our kids back to school,” he said.
While Mr. Trump has said he will not impose a nationwide mask mandate, he urged Americans last week to wear coverings, saying during a White House press briefing “they have an impact.”

More than 142,000 people in the U.S. have died from the coronavirus, the most of any country in the world, according to data compiled by Johns Hopkins University. Currently, the number of deaths per 100,000 people in the country is 43.30, 10th highest in the world. The official death rate for positive cases is hovering around 3.6 percent, though the Centers for Disease Control and Prevention (CDC) this week said the actual number of cases in the U.S. could be up to 10 times greater than the official number.
With more than 3.9 million confirmed cases of the virus, the country could pass 4 million cases by the end of the week. No country has more cases than the U.S.
Gottlieb’s comments come after Trump switched his tone regarding the pandemic on Tuesday, saying that it will probably “get worse before it gets better.” The president also encouraged all Americans to wear masks in public. Trump has previously said he doesn’t mind wearing a face covering, although he has rarely been seen doing so.
The former Trump administration official noted that the U.S.’s objective should be to limit the transmission of COVID-19.
“We have the specter right now of parts of this country where the local school boards are making a decision to close schools but the bars and restaurants are still open,” Gottlieb said. “We need to make a decision what’s important to us and what we’re willing to sacrifice right now until we get to the other side of this.”

]]>Washington — As school districts weigh whether to reopen classrooms for in-person instruction in the coming weeks, Dr. Scott Gottlieb, the former head of the Food and Drug Administration, warned there could be a surge in reports of post-viral syndromes that may be tied to the coronavirus, just as children prepare to return.
In an interview with “Face the Nation” on Sunday, Gottlieb said illnesses such as pediatric multisystem inflammatory syndrome, clusters of which have been reported in the United States, are currently being investigated. Gottlieb said one study from the Centers for Disease Control and Prevention estimated the illness occurred roughly 25 days after children displayed symptoms for the coronavirus.
“Given the fact that we’ve seen a spike in infections among kids, we would expect in about two or three weeks we might see a surge in these kinds of cases getting reported,” he said. “That will be right about the time that we’re looking at sending kids back to school. So that could cause a lot of districts to become wary.”
The Trump administration has been pressuring school districts to fully reopen for classroom learning for the fall semester, and President Trump has threatened to cut federal funding for schools that opt not to, though he lacks the authority to withhold federal dollars that have already been approved by Congress.
Local and state officials, however, are weighing hybrid models of learning for students, which would include a mix of virtual and in-person learning, and have raised concerns about how best to protect students and staff from the coronavirus.
Gottlieb said that while children are less likely to get infected with the coronavirus and less likely to be symptomatic, “what we aren’t sure about is what their propensity is to spread the virus once they do get infected, and particularly when they’re symptomatic.”
“They do appear as likely to spread the virus when they develop symptomatic illness,” he said, “and that might be because they shed as much or because of their behaviors, that a child’s more likely introducing it into the home because you’re going to hug your child even when they’re sick.”
Adding to the uncertainty about the upcoming academic year is the surge of new coronavirus infections in many states. California, Texas, Arizona and Florida have emerged as epicenters of the epidemic in the U.S., and Gottlieb predicted those states will hit their peaks in two to three weeks.
But he said as those states begin to reach the apex, other epicenters are starting to emerge.
“You have to be very worried right now about Georgia, about Tennessee, about Missouri, about Kentucky,” Gottlieb said. “We’re seeing record numbers of cases, rising hospitalizations and really a shifting of the center of the epidemic potentially in the United States. And this just portends more trouble for the fall and the winter that we’re going to be taking a lot of infection into the fall that we’re never going to really be able to come down.”
In an effort to combat the spread of the coronavirus, governors have put on hold their phased reopenings and reimposed restrictions on bars, restaurants and other establishments. Governors in some states have also issued orders requiring people to wear masks or face coverings in public.
The CDC this week called on all Americans to wear face coverings to prevent the spread of the coronavirus, and its director, Dr. Robert Redfield, said Tuesday that if everyone wore a mask, the epidemic could be under control within four to eight weeks.
But Gottlieb said it may be “wishful thinking” to believe everyone will wear a face covering.
“There’s a hardened percentage of the population that just feel that the masks are some infringement on their liberty,” he said. “We’ve been unable to find some kind of acceptable medium in this country between hapless spread and strict lockdowns.”
Gottlieb said that if the public could reach a consensus about “some reasonable measures” to take, “we could potentially get this under control and keep it under control.”
“Masks are an important first step, but I just don’t see enough of the population agreeing to wear masks,” he said.

“I think that they felt they were out of the woods after that first wave passed, but this has really been a regional experience in the United State. What happened is they reopened against the backdrop what was a lot of spread, they hadn’t really crushed the virus in those states,” Gottlieb said on CBS’s “Face the Nation.”
He said the states opened “too early” in his view.
“And people became complacent, especially younger people,” he added. “They were going out, not taking precautions.”

Despite precautions to protect more-vulnerable populations and nursing homes, Gottlieb said the virus inevitability spread to those communities since younger people were getting infected. Younger people are said to be less likely to experience the most extreme symptoms of the virus, but can spread the infection even if they are asymptomatic.
Coronavirus is still rapidly spreading across the U.S., with the number of coronavirus cases on an average day increasing in the last two weeks in 43 states.
There have been more than 3.2 million reported coronavirus cases in the U.S. and 134,898 deaths, according to data compiled by Johns Hopkins University.

]]>Washington — Dr. Scott Gottlieb, the former commissioner of the Food and Drug Administration, warned Sunday that the United States is “right back where we were” when the country experienced its first peak in coronavirus infections earlier this year.
“We’re right back where we were at the peak of the epidemic during the New York outbreak,” Gottlieb said on “Face the Nation.” “The difference now is that we really had one epicenter of spread when New York was going through its hardship. Now we really have four major epicenters of spread: Los Angeles, cities in Texas, cities in Florida and Arizona. And Florida looks to be in the worst shape, and Georgia is heating up as well, and that’s concerning.”
Forty states have reported a surge in confirmed coronavirus cases as they began to lift restrictions on residents and proceeded to phased reopenings of their economies. But amid the spikes, states like Texas and California have put their plans on hold, instead closing bars and limiting indoor dining at restaurants, while mandating residents to wear masks indoors.
Despite the jump in cases, President Trump has downplayed the latest surge, saying many of those newly infected are young and at lower risk of serious illness from the coronavirus. Mr. Trump has also attributed the rise in cases to increased testing and said Thursday following the latest jobs report showing the unemployment rate fell to 11.1% and the economy added 4.8 million jobs that the “crisis is being handled.”
But Gottlieb warned that in the next two weeks, the death toll from the coronavirus — which currently stands at more than 129,000, according to Johns Hopkins University — is going to increase.
“The total number of deaths is going to start going up again as the number of hospitalizations starts to spike again, so we’re going to see deaths creep up,” he said, adding that in the next two weeks, deaths could reach more than 1,000 per day. “That doesn’t mean the case fatality rate, the actual death rate isn’t declining. But when you have more infections, even if the death rate is declining, you’re going to get more deaths, tragically.”
Gottlieb said the country has to “accept the fact that we’re in the second wave” of the pandemic and warned that there is “not a clear line of sight on how we’re going to get this under control.”
While some drugs have proven useful for treating the coronavirus, such as remdesivir, which was found to help hospitalized patients recover more quickly, Gottlieb said the federal government and pharmaceutical companies need to be planning for other drugs coming available in the fall to ensure there is investment in manufacturing and a coordinated strategy for deploying them.
“We face a hard fall,” Gottlieb said. “It’s not clear that it’s going to get better. We’re going to have epidemics that come and go across the nation in different cities. They’ll light up at different times. But we’re not going to really be able to crush this virus at this point because there’s just so much infection around. We really don’t seem to have the political will to do it.”

]]>Former Food and Drug Administration Commissioner Scott Gottlieb said a decline in coronavirus deaths even as cases spike is likely temporary.
The concentration of new cases among younger patients, and thus the decline in mortality, is “not likely to stay that way,” Gottlieb said on CBS’ “Face the Nation” Sunday. “We’re likely to see total daily deaths start to go back up again.”
In states that have imposed restrictions in response to increased cases, Gottlieb said, “the action is much weaker than a stay-at-home order.”
“I think that these states have some difficult weeks ahead,” Gottlieb added, saying that Florida in particular “looks like they may be tipping over into exponential growth.”
“In think the next weeks we’re going to know just how pervasive the spread is” in several states, he said.
Gottlieb said that the issue in many states may not necessarily have been premature reopening, but rather insufficient time between stages of reopening.
“The speed of the opening in some of these states… they didn’t really pause in between steps of the reopening for a sufficient amount of time to see if it was having an untoward effect,” he said.
Asked about steps by the European Union to restrict entry by travelers from the U.S., Gottlieb added “I think states that have crushed their epidemic are going to start to put more stringent restrictions on travel” as well, pointing to New York, New Jersey and Connecticut, which have instituted a quarantine period for travelers from states experiencing outbreaks.
“The easiest thing we could do is universal masking and I don’t know why we can’t… that alone could reverse the epidemic,” the former FDA chief added, noting that it was the best solution particularly over the next several months.
“After about 6 or 7 months we’re going to get to a vaccine or a therapeutic…. But it’s going to be a 2021 event,” he added.

]]>Former FDA Commissioner Scott Gottlieb said the “underlying problems” of racial inequity in the U.S. need to be addressed in order to stop the coronavirus pandemic which is impacting communities of color at disproportionate rates.
“I think it’s a symptom of broader racial inequities in our country that we need to work to resolve,” he said Sunday on CBS’s “Face the Nation.”
Gottlieb said the issue needs to be addressed at two levels: why there are higher rates of COVID-19 and higher death rates from the coronavirus among black Americans.
“And the first has to do with a lot of issues of socioeconomic factors, low income issues related to overcrowded housing, where people work, the fact that they have to take crowded transportation, that they work in essential jobs, that they’ve had to continue to work and didn’t have good PPE at work,” he said. “We’ve seen black communities and Hispanic and Latino communities disproportionately in these kinds of circumstances.”
The second issue, on higher death rates, he said is due to poor access to heath care, as well as “mistrust of the health care system” and “some discrimination” in health care.
“Stopping the pandemic is going to depend on our ability to take care of our most medically and socially vulnerable. We absolutely need to resolve these underlying problems to eliminate the risk of pandemic spreading of the epidemic,” he said.
CBS’s Margaret Brennan asked Gottlieb what the response should be from elected officials.
“It’s taking resources and trying to get it into communities that you know are being disproportionately impacted by the disease,” he said. “You think of people from communities that are disadvantaged. They already lack access to health care. They lack access to testing. So they’re not only at higher risk, they don’t have the same health care opportunities.”
He also said officials need to “make sure that COVID doesn’t become punitive,” and that being diagnosed with the disease doesn’t mean someone will lose their job.
“We need to encourage them to get tested and self-identify,” he said.
