Cleveland Clinic develops first of its kind Covid-19 risk calculator

CLEVELAND, Ohio (WOIO) – The Cleveland Clinic has developed a risk prediction model, a calculator that researchers say can predict your risk of developing Covid-19, which in turn can help them guide resources to those who are most at risk.

The risk prediction model was based on the extensive research of 12,000 patients in Ohio, including those who tested positive and negative; it also isolated 400 variables and subsequently used that data in statistical algorithms to produce the risk calculator.

“We wanted to generate, based on all that information, a tool at the end of the day that could be usable and accurate,” said Dr. Lara Jehi, the Clinic’s Chief Research Information Officer.

Cleveland Clinic research shows the model is about 85 percent accurate while taking into account age, race, gender, socioeconomic status, vaccination history plus various medical history.

It takes only minutes to complete the questions in the risk calculator and your percentage risk of developing Covid-19 is available immediately.

The assessment allows doctors to gauge risk and then provide guidance to those at the highest risk.

“Making sure that patients who are at risk, get followed more intensely in our home monitoring program,” Dr. Jehi said.

The prediction model will not only help doctors understand who is at risk of developing Covid but also, Dr. Jehi said, who is at risk of developing a severe case.

“We’re looking also at who is at risk at ending up at the hospital, we’re looking at who is at risk of ending up in the ICU,” she said.

An individualized specific risk assessment for everyone was the goal, but the research also provided new insight into Covid 19.

“We identified certain factors that could influence the risk of having Covid which we did not know before,” Dr. Jehi said.

Those factors will be studied in clinical trials at the Cleveland Clinic.

The Mayo Clinic this week announced it has developed a new coronavirus antibody test that could help researchers better understand the degree of immunity those who recover from a COVID-19 infection may have.

The clinic’s new SARS-CoV-2 neutralizing antibody test “measures the level of neutralizing antibodies against SARS-CoV-2, the virus that causes COVID-19,” officials with the Mayo Clinic said in a Thursday news release, noting that neutralizing antibodies “are a subset of antibodies able to independently inactivate viruses, and are associated with protective immunity against reinfection for many infectious pathogens.”

In other words, neutralizing antibodies are the proteins produced following a COVID-19 infection that will fight off the coronavirus should a person be exposed to it once more. Other antibody tests that are currently available simply show if a person has produced any antibodies following a COVID-19 infection but do not look for these specific proteins.

“It is important to note that the neutralizing antibody test is not a replacement for the existing serology testing for COVID-19, which detects antibodies that recognize the virus and serves as an indicator of current or prior infection,” explained John Mills, the co-director of the Mayo Clinic Neuroimmunology Laboratory, in a statement.

Rather, he said, the clinic’s new neutralizing antibody test “adds that next level of detail for researchers and clinicians by providing an accurate estimate of SARS-CoV-2 neutralizing activity.”

The test will help determine how effective a person’s antibodies are at neutralizing the virus, which will help researchers “identify optimal convalescent plasma donors and ultimately help assess the efficacy of anti-SARS-CoV-2 vaccines,” said Dr. William Morice, II, the president of Mayo Clinic Laboratories, in a statement.

That said, researchers studying the novel disease are still working to determine the level of neutralizing antibodies that are necessary to know if someone is no longer at risk. It’s also currently unknown for what period of time someone who has recovered from a COVID-19 infection may have immunity against the novel coronavirus.

The new SARS-CoV-2 neutralizing antibody test will be available later this month “through Mayo Clinic Laboratories to select labs, blood banks, and biopharma clients,” the medical center said.

Mayo Clinic and corporate partners on Thursday unveiled a new test that will help answer the critical question of whether people develop immunity to the coronavirus that causes this infectious disease.

The test is first in the world that will be broadly commercially available to identify neutralizing antibodies — the proteins produced after COVID-19 that will effectively fight off the coronavirus if it ever comes back. Existing tests developed amid the pandemic show whether people have produced any antibodies in response to COVID-19, but not these key proteins.

“This is more reassuring than just … a positive [antibody test] to show you have been previously exposed to the virus,” said Dr. Stephen Russell, chief of executive of Vyriad, the Rochester, Minn., company that created the test and provided it to Mayo via a licensing partnership with a second company, Regeneron. “This test is showing what level of protection you actually have.”

Vyriad and Mayo leaders cautioned that the test can’t yet give “immunity passports,” or suggest that people are no longer at risk of COVID-19 after recovering from initial infections. Nobody yet knows the amount or concentration of neutralizing antibodies that are needed in blood serum to fight off reinfection.

Research has shown highly variable levels of these antibodies in patients who have recovered from COVID-19, including as many as 5% who paradoxically have no antibodies in response to this coronavirus.

“There’s no need for that because we don’t know what levels are significant at this point,” said Elitza Theel, director of Mayo’s infectious diseases serology laboratory. However, the availability of that test will accelerate research on that question. The test is now available to Mayo clinicians and researchers and will be provided to others later in June.

Mayo and the University of Minnesota were among the first institutions in the U.S. to develop COVID-19 antibody tests, raising hopes among Minnesota health leaders that the tests could be used to identify people who have recovered and to declare them safe to return to work and move about in society without fear of infection. That hasn’t come to pass, yet, but the prospect remains.

International research in monkeys has shown signs of immunity after infection with the SARS-CoV-2 virus that causes COVID-19. Studies of the prior global SARS pandemic in 2003 showed two to three years of immunity for people who had been infected by the SARS-CoV-1 coronavirus.

Establishing the prevalence of COVID-19 in Minnesota will be critical given predictions that the virus will linger in the state and could produce a second wave of cases in the pandemic — as occurred in the Spanish flu epidemic in 1918-1919. As of Thursday, the Minnesota Department of Health reported another 453 lab-confirmed cases and 13 deaths, bringing the toll of the pandemic to 29,316 cases so far and 1,249 deaths — including 994 deaths of residents of long-term care and assisted living facilities.

While growth in COVID-19 cases has ebbed in recent days, it’s unclear what happens next, said Jan Malcolm, state health commissioner, speaking Thursday at the invitation of the U’s Center for the Study of Politics and Governance. “We frankly have no idea where it goes from here, whether it stays in this pattern.”

The Institute for Health Metrics and Evaluation in Washington offered its first modeling predictions over a second national wave of the pandemic, likely starting with an uptick in deaths in August and September. The institute now predicts 2,193 deaths from COVID-19 in Minnesota by Oct. 1.

A key biological feature of the COVID-19 virus is a “spike” protein on its surface that helps it bind with receptors and invade human cells. While other antibodies act as triggers to activate the immune system to fight infection, the neutralizing antibodies appear to bind to the virus on their own and block its ability to infect cells, Theel said.

“Because of this, they are thought to be really good markers of immunity,” Theel said.

For starters, Mayo will use the new test for neutralizing antibodies to assess people who have recovered from COVID-19 for donating plasma. Mayo is leading a national trial to see if this convalescent plasma has therapeutic benefits for patients with active and severe COVID-19 cases.

Plasma that is rich in neutralizing antibodies could be particularly effective, Russell said. On the other hand, plasma lacking these particular antibodies could have an opposite effect and result in an accelerated spread of the virus.

“You do not want to be giving people low concentrations of antibodies,” he said. “You thereby run a risk of making things worse. So having a test for neutralizing antibodies that can discriminate between low, intermediate and high [levels] is pretty important.”

The neutralizing antibody test also could help in the development and distribution of vaccines against COVID-19, once they are validated through research and made available. Theel said initial vaccine quantities could be limited, so antibody testing could assess who is most in need of protection.

Further uses will be proven with time, she added.

“It’s kind of been the story for antibody testing from day one,” Theel said. “There was a lot of excitement, as there should have been, but at the same time we didn’t really understand the role of antibody testing, and I think there was, to some extent, overexcitement. But with the neutralizing antibody test, I would view this as another step, another tool that we can use to better understand our immune response to [the virus]. Without these tools, we cannot move forward.”

Vice President Mike Pence on Sunday said that he should have worn a mask when visiting the Mayo Clinic, a reversal that came after a harsh backlash for not adhering to the hospital’s policy during the coronavirus pandemic.

“I didn’t think it was necessary, but I should have worn a mask at the Mayo Clinic and I wore it when I visited the ventilator plant in Indiana” two days later, Pence said at a Fox News virtual town hall on Sunday, nodding sheepishly.

Pence on Tuesday toured the Mayo Clinic in Rochester, Minn., where he met with hospital staff and a patient. According to reporters who were there, every other person present wore a mask.

The vice president has said since he is constantly tested for coronavirus and since the mask prevents people from transmitting the virus, he didn’t believe he had to wear one. He reiterated that belief at Sunday’s town hall.

But his decision not to don a mask at the health center was widely criticized on social media, including by many public health experts.

The Centers for Disease Control and Prevention had recommended that all Americans wear cloth face masks in public settings where social distancing might be difficult. The Mayo Clinic on April 13 announced its own policy requiring that visitors wear masks.

Karen Pence, the vice president’s wife, defended her husband, saying he was unaware of the clinic’s rule. But a Mayo Clinic spokesperson told POLITICO that Pence and his team had been briefed about the mask policy prior to arriving.

The clinic’s Twitter account had put out a statement saying the same, before later deleting it and instead posting: “We look forward to continued collaboration to develop essential testing and treatment for our patients and communities.”

On Sunday, Pence said, “It really is a statement about the American people, the way people have been willing to step forward, practice social distancing, wear masks in settings where they can’t do that.”

He added, “As we continue to practice those principles, all of us together, I know we’ll get through this.”

Vice President Mike Pence on Tuesday responded to criticisms that he defied Mayo Clinic policy by not wearing a mask during his visit to the campus, saying he complied with federal guidelines and felt it his duty to speak to workers at the facility unencumbered by a facial covering.

“As vice president of the United States I’m tested for the coronavirus on a regular basis, and everyone who is around me is tested for the coronavirus,” he told reporters, saying he is following Centers for Disease Control and Prevention guidelines.

The CDC recommends wearing cloth face masks in public to help prevent transmitting the virus to others.

“And since I don’t have the coronavirus, I thought it’d be a good opportunity for me to be here, to be able to speak to these researchers, these incredible health care personnel, and look them in the eye and say thank you.”

Instructions on the Mayo Clinic website request that all patients, visitors and personnel bring and wear a face mask to help prevent the spread of the coronavirus.

In a tweet eventually deleted but captured in screenshots and by the Internet Archive’s Wayback machine, the Mayo Clinic wrote that it had “informed @VP of the masking policy prior to his arrival today.”

The Mayo Clinic visit is one of two trips Pence will conduct this week as the White House seeks to ease back into travel.

Trump himself has not publicly worn face masks during press briefings and other events, saying earlier this month: “I just don’t want to wear one myself. It’s a recommendation. They recommend it.”

“I don’t know. Somehow, I don’t see it for myself,” Trump said during an April 3 coronavirus task force briefing.

MINNEAPOLIS (AP) — Vice President Mike Pence has an appointment Tuesday at Minnesota’s Mayo Clinic to learn about a new coronavirus testing “moonshot” that has the famed clinic partnering with the state and its flagship university to quickly boost the state’s capacity to 20,000 tests a day.

It’s an approach that leverages a health care infrastructure not all states can match. And it should help Minnesota become one of the most aggressive states at testing on the scale experts say is necessary to safely reopen the economy. Minnesota is one of several states that have quit waiting for the federal government for help.

Democratic Gov. Tim Walz unveiled the state’s partnership with the University of Minnesota and Mayo last Wednesday, promising that every resident with symptoms of the coronavirus can get tested once the plan ramps up in the next few weeks.

“This is not a state that’s just going to get through COVID-19; this is a state that’s going to lead this nation and the world out of this,” Walz vowed of his ”moonshot.”

Pence recognized Walz’s efforts at a White House briefing Thursday, where he also praised Ohio Gov. Mike DeWine for an initiative to boost his state’s testing capacity to 22,000 per day through a partnership with reagent manufacturer Thermo Fisher. The vice president also cited Iowa Gov. Kim Reynolds for a partnership with Utah-based startup Nomi Health aimed at increasing her state’s lagging testing capacity by 3,000 per day from its current 1,000-2,000.

“This is the kind of approach and commitment that we need, and I’m glad the vice president is shining a light on promising efforts. We need more,” Jennifer Nuzzo, an epidemiologist at the Johns Hopkins Center for Health Security, said of Pence’s visit to Mayo. “Kudos to Minnesota for being a part of that.”

Minnesota has one of the lowest confirmed cases per-capita in the country, but its testing has lagged behind what the Walz administration says is needed. An Associated Press analysis of data from the COVID Tracking Project shows that Minnesota now ranks among the lowest states in per-capita testing, sixth from the bottom at 10.51 per 100,000 residents, with Kansas last at 9.01. Rhode Island is the highest at 50.51. while New York, the hardest hit state, ranks second at 41.21.

“Minnesota is probably low in terms of tests per thousand compared with other states, but the good news for Minnesota is the testing that it’s doing relative to the cases it’s finding is relatively high … ,” Nuzzo said. “You don’t want the majority of your tests resulting in cases because that suggests there’s many cases out there you’re not finding.”

Minnesota’s death toll from the coronavirus rose by 14 to 286 as of Monday, while 214 new confirmed cases raised the statewide total to 3,816. State and private labs had conducted 61,268 tests as of Monday,

There is huge variation across the country on how aggressive states have been about testing, said Dr. Joel Shalowitz, an adjunct professor at the Feinberg School of Medicine at Northwestern University. That’s partly because some states that do a lot of testing, such as New York and New Jersey, are among the hardest hit states, while some states that do less testing also have fewer cases. So a lot of testing is driven by the need to test, or the alleged need, he said.

While the University of Minnesota and Mayo Clinic are “absolutely world-class institutions,” Shalowitz said, much will depend on how accurate their testing proves to be. False positives can force healthy patients into isolation, while false negatives can lead to infected people spreading the virus. Accuracy among other institutions varies widely, he said.

“If Minnesota can increase its capacity to 20,000 tests per day, that would be a very meaningful increase in its testing capacity,” said Dr. Thomas Tsai, a health policy researcher at the Harvard Global Health Institute.

Being home to both the University of Minnesota and the Mayo Clinic “does increase the tools in the toolbox for being able to perform tests,” he said. But test numbers aren’t the whole answer, he cautioned.

“Testing capacity is one part of the puzzle,” Tsai said “The most important piece is the strategy around testing — not waiting for people to show up. You have to do active contact tracing and active outreach into the community.”
